Holland Bloorview is partnering across the health-care system to make the transition from pediatric to adult care safer and more coordinated – reducing missed care, worsening outcomes, emergency department use and isolation. On March 30, 2026, the hospital co-led a first-of-its-kind medical transitions think tank to design pathways with earlier planning, navigation support, better information-sharing and multidisciplinary models of care for adults.
